What Is Chronic Disease Management?
Chronic Disease Management refers to the structured care plans developed between you and your GP to manage long-term conditions. The goal is simple: to keep you well, prevent complications, and help you maintain a good quality of life.
If you have a condition that has lasted, or is likely to last, 6 months or more — such as heart disease, COPD, osteoporosis, or depression — you may be eligible for a Medicare-funded CDM Plan.
